How to Add a Newsletter Registration Form to a Page or Post

I want to be able to add a form that collects name, address, and email address for anyone interested in receiving our newsletter. I would like this form when submitted to send an email to our newsletter editor so they can put this person in their mailing list. Anyone know how to accomplish this or of a plugin that will accomplish this? I have seen some plugins that require a paid emailing service. I do not want that. Thanks for any help.
