shortcode [wordpress_file_upload] not working

It might be me that is doing something wrong but I installed the wordpress fie upload plugin, copied the wordpress_file_upload directory inside wp-contents/plugins directory then activatged the plugin. I then created a page in wordpress and inserted the shortcode [wordpress_file_upload] in the text field but I do not see anything that leads me to believe it is working. I thought I would see something close to the screen shots but Im presented with a blank page. Thanks for any help you all can give me.
