Pending Review Post by Members Disappeared!

I run a paid to write site, before a member will be granted an “author” status or role, he need to submit a post first for review, but there’s two instances that 2 members almost lost their post because after they submit the posts for pending review, we were notified but we can’t see it on the back end, we cannot see it on pending for review post. What is the possible cause of the problem? Right now, I use another way for users to submit a post for review, but how I wish this could be solved out.
