Some forum notifications not sending – but failure seems random

We’ve got a private forum setup for communication between a group of 80 members with varying levels of permissions. We’ve had a couple of incidents recently where no notification has gone out for forum posts, but it seems random; my test posts will go out fine, and most member messages too, but then the next day I’ll get an email from a member saying their post didn’t generate an email. I’ve tried copying the text of their post and sending it at a different/admin permission level, but my test post of their content also didn’t go through. This makes me think that the messages are being flagged as spam, but I’m not sure.
I’ve got version Version 1.4.2, bbPress Version 2.5.3, and am also running bbPress Pencil Unread, GD bbPress Attachments, and User Role Editor (just in case there are any known bugs with those plugins).
