Add Gallery/Images is blank page

Hi,

I have a problem when I click the Add Gallery / Images link I get taken to the Add Gallery / Images page (title is showing) but the rest of the page is blank with no options to upload anything, any ideas?

Everything else with the plugin seems to be fine
