Nevermind, I fixed it.
I had originally installed this locally but never activated it. I waited until I moved it to a live site, but something went wrong. Well, I went back to my local install and activated it and it did work. I compared the two WP_OPTIONS tables and found 4 missing entries:
1293 disqus_forum_url
1294 disqus_api_key
1295 disqus_user_api_key
1296 disqus_replace
I simply inserted these with their values that were listed in my local install and as soon as I clicked to configure Disqus, it went right into the login screen with the banner.
I hope this will help anyone else who needs it! 🙂
