Legitimate user being repeatedly blocked

I am having a problem with a specific legitimate user being repeatedly blocked. I added both her email and IP to the whitelist but her IP address keeps changing so it’s not keeping her from getting blocked. She is getting blocked due to “cached bad IP”. What are all the states that would cause this block?
I’ve removed the minimum time to submit forms in case it is related to automatic password filling and have also turned on logging to a history file to try to get further information. She has been using the site successfully for 6 days now and now every time she tries to log in it gets blocked. Anything else I can do to get her unblocked? She’s a paying customer and this makes me look bad.
I would really like to be able to specify user accounts that shouldn’t be blocked, not just IPs since IPs change all the time with some providers. This plugin has been great so far for stopping spammers but I can’t have it stopping legitimate customers having access to their purchases.
