When I enable “Use a Red Herring form” setting (which works very successfully when enabled) I run into problems with unregistered users replying to comments: the comment is put on the top-level of the comment hierarchy (as if it was not a reply to another comment).
If you use hierarchical comments, it does break the list. I am looking at this, but I have yet to figure out why. I will try to get this into the next release.

When I disable the setting, everything is normal.
