[Plugin: Simple Modal Contact Form] SMCF hyperlink not bring form up

    Hi there,

    I have installed SMCF correctly and activated it. I have added the function to my page template correctly, I can see the form HTML that has been added to the source when I test it. Unfortunately, when I test it the contact form is not over-layed on the page, I am taken through to my contact fall back page. I have JS enabled and am using the Boilerplate theme.

    Has anyone any ideas why it wont work?
