Maybe you can try this addon: http://wordpress.org/plugins/contact-form-7-style/ . Right know it doesn’t have many options because it’s only the first release, but I think in the next release you can do almost anything you want with a form that is generated with Contact form 7. Sayng this because i’m one of the authors of the addon plugin.
If it’s not enough for you, you should follow our updates. 😉

Yes you can also sue it as background, like other css properties, for image you can upload it to media section.

You did a great plugin, its just simplified the process and anyone can use this. I have worked on many sites and I always use custom coding, but now onwards, I’ll use your plugin, I tried it and it works great.
