I installed this plugin and loved it the first couple of days.
Then suddenly, Defensio was infested with 150 comments in two hours in it's quarantine, all linking to: http://www.cba.edu.kw/gne/forum/topic.asp?TOPIC_ID=10487
Definitely, some one has figured out how to hack this plugin.
Sorry man, you've to go back to the drawing board !
Math Comment Spam is a nice plugin that stops infesting manual comment spam. If you're getting spam in form of "PBs & TBs", it cann't help, you've to install some other plugins to stop this form of spamming.
You can install Simple Trackback Validation plugin to stops TBs.

I know that you are talking about Rok and thank you for your reply.
I am aware of Trackback and Pingbacks but I assure you that the amount of spam I mentioned is actually comment spam !
I hope the plugin-developer is working on someway to make it more fortified.
In the meanwhile, I've installed "Simple CAPTCHA" by zorex which is providing some much needed relief !
