Cant get categories to look ok

My setup involves different regions, and then within these regions I was going to enter the sub-category for all the services with the companies able to use this when listing.

But it seems I have only 2 options, one is to show everything in directory which is all regions, all services etc which becomes a massive page what is very hard to comprehend, or if I show only parent categories, it only shows the top ones, and the other ones are never seen, even if visitor wanted to click on region, they don’t then get to see the sub-categories under this region etc.

So unsure if it’s something I am missing here, or if this is only 2 ways to deal with it.

In a perfect world when person clicks on business directory, it would show the top level categories, and when person clicked on one of these i.e. there location, inside this was then all the sub-categories. Seems like something like this is not possible.
