HTML and Web Design is probably beyond the scope of this support forum.
Many people, like I, first found a theme whose structure is similar to what they envisioned, and used that as a starting point of their creation. This works great IF the theme is not copyrighted and IF the theme validates.
Others who start from scratch read http://codex.wordpress.org/Theme_Development first.

It sounds like you’re not necessarily looking to design WP themes, but wanting to know how to design in general. In that event, I would highly recommend reading some of the more well known and helpful design websites — especially those with a high inspiration threshold — such as Mezzoblue, Web Designer Wall, 456 Berea St, Jason Santa Maria, et al. Also, become familiar with a graphics program such as Adobe Photoshop or Illustrator.
Finally, make sure you have a firm understanding of the basics of HTML, CSS, PHP, and JavaScript. (The last is optional, but highly helpful.) There are plenty of great books on the subjects.

ok i have another question i have me a blank template im using to learn with but it came with a left side area for ads how do i rid of that and put my own catergory section there?
i got that figured out but got a diff question i wanna add a scrolle marquee type code in that area that the ad wouldve been in but the code says to put half above the head section of html and hald in the body section how would i do this for php files?
I used this guide and it worked out well. Made a new theme from scratch and it came out quite nice.
cool thank you i am having another issue i cant seem to get the footer to change to my own typing its encrypted so is there a way around this?
