Same question here. I've got thousands of images in the "old school" format, and it would be nice to have them in the new gallery setup.
Same issue. Here's an example:
I have a lot of images in the root of the Uploads folder, but only images that are inside the new upload structure: wp-content/uploads/2008/current month will appear in the Media Library. Darn :(
I tried to manually create a folder for 2006 and a subfolder for january (01). Then via Dreamweaver I uploaded an image, but it didn't show up in Media Library.
What to do? I would really love to clean up the root of my uploads folder.
Help.

electrolund - Not the way I wanted it too. You can't put a custom time & date stamps on the images you upload through the Add From Server plug-in. Not from what I know though.
It's just important for me, that the old images I put into the new folder structure, can have their original time & date, so they won't appear as newly uploaded, if you know what I mean.

Surely this must be fixed somehow. There must be a lot of people with this issue and the desire to clean up the root of the content folder.

I just manually placed all the old uploads in a custom folder: wp-content/uploads/uncategorized and went through all the post to correct the links. Repetitive work, but now my wp-content folder is nice and clean...
