Can't Add Media to Post/page with WordPress 3.6

Lost add media functionality with the 3.5 update and was using the bulk attach feature as a work around. Lost attach functionality with 3.6. Changing themes is unfortunately not an option.

Gone through the first wave of trouble shooting

1. Deactivated ALL of the plugins.
2. Manually emptied the browser cache.

Also read through the post below that seems to have the same issue, but still unable to fix.
