Visual Editor Missing Plugin Buttons

Hello, I have tried everything I can think of to get my plugin buttons to show inside of the POST Visual editor. All WordPress TinyMCE Advanced buttons show fine but not any buttons that are associated with plugins such as WP Utlimate Recipe, WP Video Embedder, Recipe Card, Etc. Any plugin that puts a insert button into the visual editor do not show up. It is this site only. I have other 3.5.2 sites that work fine. However, I have tried disabling each plugin, changing themes, copying over wp-admin and wp-includes folder, reinstalling 3.5.2 through update-core.php, etc etc.
