W3 Total Cache may very well be the reason.
We’re working on a new version, would you be willing to test it, to see if you still experience this issue ? I’ve changed something in how the images get retrieved in the new version and it might just have fixed this issue as well (not sure though, not a set-up I have tested).
You can find the new version on Github.
Alternatively you might want to try turning on async mode. That might fix your issue just as well.
