How do I activate this plugin to work on my 'ask a question' menu button?

    I’ve downloaded and installed the ‘ask a question’ plugin but no form appears on that page when you click the menu button. In other words, there’s no form to leave a question on that page nor a ‘submit’ button so that I can get a question sent to me via email.
