Can anyone recommend a plugin…

I’ve been asked to develop a page on a wordpress site that users can post into a few fields, ie. Name, Age, City, Idea, etc. and then when they click submit it displays the results below on the same page, at the top of the list of submissions…. very similar to how the comments function work, but with the ability for me to customise input fields and how the output is displayed.

What plugin or method will get me closest to what I’m after?
