Slide out/up survey form?

Hi guys,

I like Qualaroo but am not willing to pay $80/month. What is the best free/low cost plugin that offers a similar function? Having a small box at the bottom or side of screen that says something like ‘Feedback’. When the user hovers it slides out with custom questions that they can complete which will then either be compiled into the Dashboard or emailed to me.
