I know that changing permalink structure on an established blog can wreak havoc on search engine results. I have a two-year-old blog with lots of categories that have built up over time. I'd like to rename and rationalize the category name structure, but before doing this want to ask if there are any downsides (especially from SEO/SERP standpoint) of doing this.

It might depend on the fact whether the categories were part of your custom nice permalinks or not.
I agree -- as long as you arent using category names in your permalinks, I wouldnt worry. Ive re-organized my categories at least 2x in three years and have never seen an adverse effect. Page rank just keeps going up.
