Email subscribe plus coupon plugin?

So I am looking for some code or a plugin that will do the following

1: Let people subscribe to a email list, this only has to collect the email and name
2 Send them a coupon to print out with their name and a number so we can keep track of them

If the code/plugin does not send out the emails I am ok with that as we will be making update emails on the fly and sending them out about once a month.

The hole goal is to collect emails address and in turn give them a coupon for x-amount off.

Anyone here seen something like this?
