Not submitting

Hi – I am trying to use the plugin on Genesis Generate theme. I have it all set up correctly (i think), but when i hit submit, it’s not actually submitting.
Have been speaking with Nick at Studiopress and after investigating himself, he suggested I contact you directly.
I have deactivated every plugin on the site also and it’s not a conflict. I do get a redirect URL, but it doesn’t seem to be processing the form in the output.
