Custom Contact Form

Hello,
I’m searching for a plugin that allows me to create a custom contact form. I need more fields than offered by Wp-ContactForm and Secure and Accessible PHP Contact Form. I tried installing Intouch, but it crashed the site with the following error (see below), so I had to delete the intouch files from the plugin directory to restore the site. I was never able to even play with it. Any ideas on plugins that would allow a novice to create a custom form or ideas on how to get intouch to play nicely are appreciated.
