Second Form, Did not receive any email

Hi guys I am having problem with this plugin, plugin is great.. but now having issue.

i have 2 forms. first form is sending email..

But on second form.. after clicking the button to send. the status text says… Sent… but i didn’t receive any emails for the second form.

Please help.
