Adding multiple response fields to a registration page

We’re trying to add fields to our registration page, but can’t seem to get multiple checkboxes to work using the plugin. These are the questions we’d like to have new users answer optionally, but have been unable to implement more than one checkbox. Is there a way to make this work…

Here are the questions we’re trying to ask:

I listen to WhiskyCast episodes using: (checkboxes for iTunes, the WhiskyCast web site, and one for other podcast services with a text box for them to name if that option is checked)

I listen to WhiskyCast Virtual Tastings episodes using: (same options plus “I don’t listen to WhiskyCast Virtual Tastings”)

I watch WhiskyCast HD episodes on: (The WhiskyCast web site, YouTube, iTunes, Other Online Video Source (with a text box as above)
