Did you check your picture URL (right click on the broken image, copy URL and check the path)
the domain is doubled I don’t know why.
OK try with
$avatar_url = $avatar_src;
are you using WP MU or single?

I had the same problem.
I replaced:
$avatar_url = plugins_url('/user-avatar/user-avatar-pic.php')."?src=".$avatar_src ."&w=".$width."&id=".$item_id."&random=".$file_time;
With:
$avatar_url = $avatar_src;
Reuploaded, cropped, and was up and running. Dev may want to consider patching.
i crop the image and then just show me a blank square.
my problem was the plugin user role, i unmark upload files for all the users
