I have been having ongoing issues of add media for post or pages not inserting into pages or posts without manually doing it
Please let us know more specifically what happens when you try to use the Media Button in the editor…no errors show? It all completes but no image shows? Does an image placeholder show on post/page?
Have you reviewed what your upload file size limit is at host (or total hosting allotment is?)…What env are you running in? Any plugins? What Theme?

Essentially I’ll use the media button, no errors appear and no placeholders appear either. But when I manually add the code with the image url, its fine. I am currently using ink theme from themetrust, and my web link is mustardbuffalostudio.com
I’d suggest checking with the theme developer as it looks like that theme may have compatibility issues with WP3.5+ — at least it only says 3.4+ on this page :

I have installed ink theme and cannot seem to get rid of the margins that appear between each thumbnail image on the first/portfolio page – despite trawling through the css and altering various margin/padding settings (I only know a little CSS at the moment)….can anyone help?! On the demo all images are flush to each other?
