    Hi,

Yes Gravity Forms (Developers Edition) and GF PDF Extended can achieve everything you require. GF will do point 1 and 4 and PDF Extended 2 and 3.

If you have the development knowledge you can set everything up yourself with the help of our online documentation. Alternatively, you can purchase one of our business packages, email us the details and a draft template in Word or PDF format and we can complete the work for you.

My server is down at the moment (which includes the plugin website) so if you have any questions please feel free to email me on blueliquiddesigns[at]gmail[dot]com.

    Hey cbriere,

Did you every get your form working the way you wanted?

    Hi WJ,

He’s currently in discussions with me about using our Business Plus Package to do what is required. You could also do it with the developers version with a bit of know how.
