Plugin: Contact Us (different departments & locations)

I’ve looked threw the plugins and could not find any that could do this, so now I’m turning to the awesome community here to see if this plugin exists.

A company I am working with has multiple locations, so we’d like to have a contact forum that 1: allows you to select the proper locations & 2: once the location is selected allows you to choose service or sales.
