Auto Tweet/Mention Insert Hashtag Plugin

Hey guys, so I was wondering if there are any plugins that not only autotweet the title of your posts, but also have a function where you can add in a mention of someone on twitter. So I run a hip-hop blog, say the title of the post was “Kanye West — Song #1 (Video)”. How could I make it so that it would tweet “Kanye West — Song 1 (Video) via @supercoldchi @kanyewest #song1 #kanyewest #chicago”
