Can anybody suggest a Plugin for a DB

I wish to create a number of forms. The forms should have a unique primary field and be able to store a range of different field types. A copy of the form should be sent to the originator. I then need a GUI to view, modify and approve the sent forms, able to send emails to one email account or to a group of emails, and do a search. Could do this in the old MS Access DB years ago.

What would you suggest? it is only for a very small site, But I would pay for a good solution.
