I have used the shortcode format below (NextGen Gallery 1.910)

[nggallery id=12 template=caption]

but am getting no thumbnail or lightbox type captions.

Any suggestions?

Just tried, no captions.

Make sure you goto Manage Galleries and you have a Description and Alt & Title Text / Description filled.
