Instead of manually emailing a commenter and then copying the reply to the blog comment form, you can do BOTH simultaneously. Simply click an icon on the comment, type your reply, and the comment is logged on the blog AND emailed to the commenter.
If the commenter has checked the box for the “Subscribe to Comments” plugin, my plugin does nothing, to prevent duplicate emails.

I’ve just released a new WordPress plugin called Comment Email Responder, written up on my blog as well.
