no "next" button

I created a survey using the Simple Survey plugin (extended version). When I completed the survey I inserted the code [wp-simple-survey-1] into a blog post to test. The problem is that there is no “next” button inside the form, just a “back” button which you obviously can’t click on in the first question. There is a “next” button that appears outside of the survey form and to the right. If you click on it, it performs as the “submit” button.

How do I fix this?
Did I miss something in the setup? I keep looking through everything to see if I missed clicking a box or some step along the way but can’t figure it out.
