Form to get results from database back on page

Can a form be created using html and php to select some criteria then get the results from a connected database back on the page. Can someone point me to instructions for this. I have the code for my form, but how don’t know how to make it work within WordPress.

The form would be something like:

Select: Radio buttons – Boys Names, Girls Names, Both
Origin: Select Box – List of Countries
Meaning: Text field
Names Begins with: Text Field

I’m also trying Form Maker plugin to create a form to connect to a database and give results back on the page – waiting for response from Form Maker forum – not sure if it can be done.
