How to add custom htaccess

I wanted to add in this 5G Blacklist to my records:
http://perishablepress.com/5g-blacklist-2012I tried adding this via a .htaccess file that I upoaled but all I got was a bunch of blank 403 records.
Can I add this to the the Raw htaccess? or can I create a new module and then create a .5ghtaccess file? What is the correct method?
