Post & Page Notes, shortly PPNotes is a plugin to simply at a header and footer to your posts and pages, without editing your theme.

Download Version 0.1.1

Extract the zip file and just drop the contents in the wp-content/plugins/ directory of your WordPress installation and then activate the Plugin from Plugins page. Then go to 'Settings' and click 'Post & Page Notes'.
