Supple Forms
- Upload the plugin folder, supple-forms, to the
/wp-content/plugins/directory - Activate the plugin through the 'Plugins' menu in WordPress
- Make sure there was no message warning you that the tables were not created. If your WP database user does not have appropriate rights (this should be very rare), the tables may not be created
