1. Install Self Hosted Plugins directly from the repository like you would any other plugin

When you activate the plugin for the first time, it will automatically install the extra tables in the database that you need. It also creates your list based on the name of your blog and the admin email. It also sets up some frequently used fields (name, address, phone, etc). Go to poMMo > Setup > Fields to customize this list. I recommend keeping the "First Name" and "Last Name" fields as they are required by the poMMo Plus add-on, which offers a greatly improved admin experience.
